1. I often feel spiritually unfulfilled despite my active involvement in church activities and positions.
2. I feel a bit uncomfortable but don’t think it’s anything serious being in a place where materialism, prosperity and application of bible to enhance social-economic life is the focus.
3. I hate man-worship but think my pastor, his message, and ministry have the exclusive correct doctrine and message compared to all others.
4. I notice that my spiritual passion, zeal for the Lord, and prayer life aren’t as strong as they once were.
5. Because of the hype, noise, and other factors, I find it challenging to discern between emotional worship experiences and the true presence of the Holy Spirit during praise and worship sessions.
6. I struggle to hear or recognize the voice of the Lord clearly, but I adequately compensate by faithfully relying solely on my pastor’s and church's guidance.
7. I feel a bond with my current church or denomination, even though I sense it is no longer meeting my spiritual needs, fulfilment, or where I am supposed to be.
8. To be safe and not go into error, I tend to prioritize and stay within the teachings and words of my leaders over personal study, application, and revelation of the Bible.
9. I feel a bit hesitant to leave my current church because of the social-economic benefits and advantages I need and it provides, though I feel a bit spiritually unfulfilled.
10. I do not belong to any specific church because they all have one issue or other but in order to be within Church I move from one to another without commitment to any but Jesus.
11. I have distanced myself from the church altogether due to disappointment or a feeling of spiritual exhaustion.
12. I feel my church is overly focused on extra-biblical doctrines or practices that don’t align with scripture, but I’ve been too long and put in too much to leave.
13. I have trouble trusting any church leadership due to disappointment in the personal life of known leaders.
14. I long for genuine fellowship with believers where Christ alone is the focus, personal spiritual growth and holy spirit is emphasized more.
15. I am not connected to any church, ministry or spiritual leadership and I do not have any formal spiritual covering over me, and it doesn’t bother me.
16. I have an honest concern that my enthusiasm for material blessings often overshadows my desire for spiritual growth
17. I prefer sermons and teachings that make me feel good rather than those that challenge me spiritually.
18. Culture, social-economic and leadership consideration are top priority for my denominational affiliation.
19. As a worker in the church, I love to participate in Christian activities, and other charitable events in an effort to keep my faith, grow the church of God, and please God so as to make heaven.
20. Sometimes, I think I know more than my pastors and leaders and I often correct them secretly in my heart, but I am not ordained to preach or teach
